Consent & Declaration – Health Ashram

I hereby voluntarily consent to share my personal information, including photographs and/or videos, with Health Ashram (Swasth Ashram). I understand and agree that such content may be used for the purposes of raising awareness, spreading knowledge, improving the quality of teaching and training, documentation, research, and for educational or promotional activities related to Health Ashram.

I confirm that I have carefully read, understood, and agreed to all the policies, guidelines, terms of use, and services of Health Ashram. I am participating in Health Ashram programs, sessions, consultations, or activities entirely by my own free will and decision, with the intention of gaining knowledge, awareness, and wisdom to improve my health, fitness, well-being, and overall quality of life.

I clearly understand and acknowledge that Health Ashram is a wellness and social development initiative and not a medical institution, hospital, or treatment provider. I understand that the guidance, teachings, practices, and recommendations provided by Health Ashram are educational and lifestyle-based in nature, and are not a substitute for professional medical advice, diagnosis, or treatment.

I agree that I shall be solely responsible for my personal health decisions, actions, and outcomes. I hereby release, indemnify, and hold harmless Health Ashram, its founder, board members, staff, volunteers, mentors, facilitators, well-wishers, and associated persons from any liability, claims, demands, losses, or damages arising out of my participation, whether foreseeable or unforeseeable.

I further declare that I shall not initiate or pursue any legal action, claim, or dispute of any nature against Health Ashram or any person associated with it, for any reason whatsoever, at any time, arising directly or indirectly from my participation in Health Ashram activities or services.

I understand that signing or submitting this consent is mandatory before becoming a member of Health Ashram or booking any appointment, session, or program.
